Application for students Accident assistance scheme under SWF Karnataka

Karnataka state Students Welfare fund ....
In short form called as SWF,under this scheme department announce the relief fund to students who suffers from accident or death..
Conditions
1. Submit application within a year
2. Mention bank account details
3. Applicants self statement regarding relationship.
4. Submit age proof.
5. Submit supportive documents like Aadhar, ration card, FIR, post mortem report etc if necessary
6. Submit doctor certificate and hospital bills.

Details. & Max relief fund
1. Normal death on accident - RS 50,000
2. Total disability in accident -Rs 25,000
3. Partial disability in accident -Rs 15000
4. Normal death RS 10000
